Because you've set things up wrong or maybe because you're using broken
software.

For Sendmail the critical information to include in a question is:
1. What OS you are running on.
2. What version of Sendmail you are running.
3. If you are running any milters, their names and versions.
4. The .mc file from which your operational sendmail.cf was built.
5. The output of sendmail -d0 (i.e. the build settings.)
You might be able to get away with a subset of those if you understand
your problem well enough and can communicate the content of that
understanding in your post (aassuming you have it right...)
